* Absolutely NO ABBREVIATIONS. - County, City, State or any other must be spelled out in thier entirety.

* Form MUST BE SIGNED by a parent or legal guardian only.

* DO NOT FILL OUT THE BOTTOM SECTION of the form, that area is for the pilot information.
